Major Responsibilities:

  • Prepares flower beds, by hand digging or rototilling and raking, for planting; prepares plants and maintains flower beds, horticultural displays and plants in greenhouses.

Key Qualifications:

To autonomously diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures for residents in the Long-Term Care Homes and Services Division within their legislated scope of practice.

 

Major Responsibilities:

  • Delivers primary health care to residents in the Long-Term Care Home in the management of acute and chronic medical conditions, therapeutic management, health promotion and disease/injury prevention in order to deliver comprehensive health services.
  • Conducts focused health assessment using and adapting assessment tools and techniques based on resident needs.
  • Completes health history into resident's situation, including physical, psychosocial, emotional, cultural and ethnic dimensions of health.
  • Performs physical examination and identifies and interprets normal and abnormal findings.
  • Orders diagnostic investigations, and interprets results using evidence-based clinical reasoning.
  • Synthesizes health information using critical inquiry and clinical reasoning to formulate a diagnosis, health risks and states of health/illness.
  • Communicates with residents about the health assessment findings and/or diagnosis, including outcomes and prognosis through the application of knowledge of pathophysiology, psychopathology, epidemiology, infectious diseases, behavioural sciences and family processes when making diagnoses and providing overall therapeutic management.
  • Selects appropriate interventions from a range of non-pharmacological and pharmacological interventions to restore or maintain resident functional, physiological and mental stability to achieve optimal health.
  • Promotes safe resident care by mitigating harm and addressing immediate risks for residents and others affected by adverse events and near misses.
  • Participates with the Medical Director and other senior management staff in the development and implementation of evaluation processes and the identification of strategies to continually improve clinical outcomes and effectively manage risks.
  • Collaborates with senior management in the Division and other community health services in initiatives for the development, implementation and evaluation of systems to promote continuity of resident care across the continuum.
  • Ensures Accreditation Canada standards are evident in daily practice.
  • Ensures resident files and health care records are properly maintained in compliance with applicable legislation and divisional expectations.
  • Identifies, and collaborates with the Medical Director on, educational programs for the care team.

 

Key Qualifications:
Your application must describe your qualifications as they relate to:

  1. Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practise.
  2. Experience as a Nurse Practitioner in long-term care, acute care, rehabilitation, continuing care or primary care.
  3. Experience in long-term care, with providing empathetic/relational care for the elderly and those living in long-term care, and clinical work experience with frail older adults, critical care, and mental health as a Nurse Practitioner.

About This Role

POSITION OVERVIEW

 

The Human Resources Business Partner (HRBP) ensures that people strategies, programs and processes across the organization are aligned with the priorities and goals of the business unit or function. The incumbent acts as a liaison between the organization and HR functions in a context of organizational transformation to tailor the design and implementation of HR programs and services in line with the business needs of the business unit or function. They coach leaders on how to embrace change and get the most out of their teams. They work closely with HR stakeholders to improve organizational design and structure, provide change leadership, talent management and coaching to leaders and employees.

 

CHALLENGES TO BE MET

  • Support leaders on all people and talent topics impacting business groups, bringing forward their market view/understanding to set HR prioritization and pace, managing/aligning multiple initiatives, including delivery of talent cycle activities

  • Build and strengthen trust with leaders, employees and key stakeholders as a trusted advisor, i.e., someone who is informed, thoughtful, creative and able to leverage HR and other stakeholder centers of expertise as needed to provide value-added advice and recommendations

  • Understand external trends/market, which entails having deep knowledge of the organization and its culture, and use data, metrics and people analytics to provide business leaders with key insights and influence organizational, talent and diversity strategies.

  • Act as a change agent, a positive thinker, and a critical business thinking partner to anticipate, shape, and influence organizational evolution, describing how to structure and evolve teams

  • Support leaders through coaching

  • Be a champion of a culture of respect and promotion of a diverse and inclusive workforce at all levels of the organization

  • As a key partner to the broader HR team, provide key contributions to the continuous improvement of HR processes and programs, and influence to align business and HR team programs

 

WHAT WE ARE LOOKING FOR 

  • University degree in a relevant field; Master's degree considered an asset

  • Must have a minimum of 8 years of experience as an HRBP with in-depth knowledge of multiple HR disciplines including compensation, organizational development, change management, employee relations, diversity, equity and inclusion, performance management, training and recruitment

  • Excellent communication skills, oral and written, in English. French is a strong asset

  • Business acumen: understanding of external trends and the marketplace

  • Strategic thinking, influencing, organizational and interpersonal skills

  • Results-oriented with a proven ability to execute quickly, efficiently, to deliver consistently and to be able to prioritize and manage

  • Experience working with leaders at all levels

  • Strong analytical skills, including the ability to generate and analyze reports

  • CHRP / CRHA designation is an asset

They perform administrative and miscellaneous duties as directed by management and serve as the shift supervisor for local personnel, as required.

Pay Rate: $31.00/HR.

Shifts: Monday to Friday 1500-2300hrs

ESSENTIAL FUNCTIONS:

  1. Monitors physical security operations at the client Datacenter. 
  2. Monitors cameras and access control systems covering data calls and entry/exit doors. 
  3. Dispatches appropriate personnel, as necessary. 
  4. Maintains logs and records in accordance with the Datacenter Standard Operations Procedure (SOP). 
  5. Identifies and escalates issues in accordance with documented policies and procedures. 
  6. Reviews all reports written during shift. 
  7. Ensures all scheduled posts are filled. 
  8. Briefs oncoming Supervisor/Team about incidents during shift or in progress.
  9. Maintains documentation and follows standardized procedures to successfully complete unannounced audits. 
  10. Collects complete information for detailed report and appropriate follow-up. Prepares well written reports and maintains strict distribution to “need to know” parties. 
  11. Cross-training will be provided for all site-based roles, enabling you to step in for other position coverage and offer relief breaks to guards, when required by operational needs.
  12. Prepares written and electronic logs and reports as required for the site while entering information in appropriate computer systems. 
  13. Read and comprehends standard operations procedures, post orders, reports, and other printed materials. 
  14. Performs additional tasks assigned by management. 
  15. Conduct training with the staff on a daily, weekly, and monthly basis.
  16. Conduct drills, and Quality Assurance & Quality Control (QAQC) review
  17. Conducts foot patrols (for long periods of time or up to 12 miles of distance. It may involve climbing stairs and walking up inclines and on uneven terrain) completing patrol checklists.
  18. Assist STM with regards to scheduling, covering any open shifts.
  19. Identifies security shortfalls and offers suggestions from improving the security program. 
  20. Evaluates and escalates potential safety issues within the facility. 
  21. Required to know and follow safe work practices, and to be aware of company policies and procedures related to job safety, including safety rules and regulations. Notify supervisor upon becoming aware of unsafe working conditions. 
  22. Identifies security shortfalls and offers suggestions from improving the security program.

MINIMUM HIRING STANDARDS:

  • Must be at least 18 years of age.

  • Must have the ability to speak, read, and write English proficiently. 
  • Must have a high school diploma, secondary education equivalent, or GED.

WORKING CONDITIONS AND PHYSICAL/MENTAL DEMANDS
